Frequently asked questions

What is lyophilized tirzepatide?
Lyophilized tirzepatide is a powdered form of the medication tirzepatide, approved for chronic weight management in adults with obesity (BMI ≥ 30 kg/m²) or overweight (BMI ≥ 27 kg/m²) with at least one weight-related comorbid condition (such as high blood pressure, type 2 diabetes, or high cholesterol) when used alongside diet and exercise.
Lyophilized tirzepatide is administered as a subcutaneous injection (under the skin into fatty tissue) in the abdomen, thigh, or upper arm. Typically, the medication is injected once a week. Never administer tirzepatide intravenously or intramuscularly.
The most common side effects of lyophilized tirzepatide include nausea, diarrhea, vomiting, constipation, and decreased appetite. Rare but more serious side effects may include pancreatitis, gallbladder issues, and low blood sugar levels. Stop using tirzepatide immediately if these side effects persist or worsen.
Lyophilized tirzepatide is not safe for everyone. It should not be used by individuals with a history of pancreatitis or severe gastrointestinal issues. It is also not recommended for pregnant or breastfeeding individuals. If undergoing surgery or certain medical procedures, your doctor may advise temporarily discontinuing tirzepatide.

Dosage

The initial dose of tirzepatide is 2.5 mg, administered subcutaneously (under the skin into fatty tissue) once a week.
Yes, the tirzepatide dose can be increased after at least 3–4 weeks at the current dose. The dose is typically increased in 2.5 mg increments, up to a maximum dose of 15 mg once a week. Increase the dose only if weight loss is insufficient or appetite control is inadequate.
Tirzepatide is administered once a week.

Tirzepatide may take a few weeks to start showing effects. However, some individuals may notice results earlier.

If you miss a dose of tirzepatide, take it as soon as you remember. However, if it is close to the time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take two doses at once.

What is the recommended duration of tirzepatide treatment?

The treatment duration varies based on factors such as:

  • Your medical condition: Whether it is for diabetes management or weight loss.
  • Your response to the medication: If benefits are observed without severe side effects, long-term use may be considered.
  • Your treatment goals: For weight loss, treatment may continue until desired goals are reached and maintained.
Tirzepatide should be stored in a refrigerator at 2°C to 8°C. Do not freeze tirzepatide once reconstituted.

Bacteriostatic water

Bacteriostatic water is sterile water containing a preservative, usually benzyl alcohol, to prevent bacterial growth. It is used to reconstitute or dilute injectable medications.
Bacteriostatic water should be stored at room temperature (20°C to 25°C) away from heat, humidity, and light.
No, bacteriostatic water should not be frozen, as freezing may damage the vial and the preservative.
Once opened, bacteriostatic water should be used within 28 days.
Proper reconstitution of the tirzepatide vial with bacteriostatic water is essential for treatment efficacy. Follow these instructions:
  1. Determine the tirzepatide dose (10 mg, 30 mg, or 60 mg).
    • 10 mg: Add 0.4 ml of bacteriostatic water (40 units with an insulin syringe).
    • 30 mg: Add 1.2 ml of bacteriostatic water (120 units with an insulin syringe).
    • 60 mg: Add 2.4 ml of bacteriostatic water (240 units with an insulin syringe).
Tirzepatide (mg) Bacteriostatic Water (ml) Insulin Syringe Units (U-100)
10 mg 0.4 ml 40 units
30 mg 1.2 ml 120 units
60 mg 2.4 ml 240 units

Administration of Tirzepatide

Tirzepatide is administered as a subcutaneous injection, meaning it is injected under the skin into fatty tissue. It is given once a week.

Dosage Conversion Table:

Dose (mg) Subcutaneous Units (Insulin Syringe 100-U)
2.5 mg (initial dose) 10 units
5.0 mg 20 units
7.5 mg 30 units
10.0 mg 40 units
12.5 mg 50 units
15.0 mg (max dose) 60 units

Tirzepatide is commonly injected in:

  • Abdomen: The most common and usually least painful site.
  • Thigh: Another accessible option.
  • Upper arm: Less common but also viable.
Injection Application:
  • Subcutaneous route: The injection is administered under the skin into a fatty area (such as the abdomen, thigh, or upper arm).
  • Insulin syringe: Use an insulin syringe to draw the correct amount of tirzepatide.
Tirzepatide Dosage:
  • Initial dose: Typically starts at 2.5 mg.
  • Dose increments: If necessary, the dose can be increased in 2.5 mg increments each time, provided there is no significant weight loss within a 3–4 week period and the medication is well-tolerated.
  • Maximum dose: The maximum recommended dose is 15 mg.
It is typically administered once a week (every 7 days).
This frequency of administration helps maintain stable levels of tirzepatide in the body, maximizing its benefits for glucose control and weight loss.

You can inject tirzepatide at any time of the day, with or without food. However, it is important to administer it at the same time and on the same day each week to maintain consistent medication levels in your body.
